The Beyond Wellness Scholarship Application can be found at the bottom of the package deal tab on our website as a PDF link. All applications will need to be turned in by October 25th at 6PM. If you would like to mail your application into us, then please private message me for the mailing address. You also have the option to e-mail them to us at beyondwellnessllc@gmail.com.
